Our remote monitoring kit enables the individual's vital signs to be taken by a caregiver, which is then recorded automatically and monitored in real-time by our team of healthcare professionals in SJMC

SeniorConnect is a remote patient monitoring service for elderly patients aged 60 years old and above, who are living independently or with a caregiver.

How to setup the gateway

Step

1. Plug the USB cable into the USB wall charger and connect the other end of the cable to the gateway.

2. Ensure the gateway is within 15 meters of the wifi modem & within 3 meters of the vital sign devices.

Plug the wi-fi modem to the USB wall charger. Ensure the indicator shows a bluish green light for the strongest network connection

How to take your health vitals?

Vital Steps

Thermometer

1. Remove the protective cap.
2. Aim thermometer 3-7 cm way from forehead.
3. Press the scan button and take your reading.

Oximeter

1. Clamp the oximeter on your middle or index finger.
2. Rest on your hand on a flat surface.
3. Keep the oximeter in place for a minute or longer to get the reading.

Blood Glucose Monitor

1. Insert a test strip into the meter.
2. Prick your fingertip with the needle (lancet).
3. Hold the test strip to the drop of blood.
4. Eject the used test strip once the reading is done.

1. Hold your arm out with your palm facing up.
2. Wrap & tighten the cuff on the upper part of your arm.
3. Place your elbow on a flat surface and relax.
4. Press the "ON" button to take the reading and remove the cuff once the reading appears on the display.

After the assessment and found to be suitable for this service, our TeleConnect team will brief you on the use of the SeniorConnect Kit.

Frequently asked questions

Is SeniorConnect available to all patients across Malaysia?
No, currently it is only available to patients in Selangor & Kuala Lumpur. But we are in the midst of expanding our horizons to cover the entirety of Malaysia & more.
Is everyone suitable to utilize SeniorConnect?
No, patients are required to either be able to take their own vital signs at home, or have a nominated carer in charge of taking their vitals.
How does a patient know if they are suitable for SeniorConnect?
A baseline assessment is required to determine the suitability of patients for SeniorConnect. (Chronic illness might require other services available at SJMC)
