Terms and Conditions

1. The Package(s) are non-refundable and cannot be exchanged for cash or any other products or services.
2. The Package(s) purchased are not valid in conjunction with any other current or future promotions or discounts.
3. The Package(s) are valid for a period of 3 months upon full payment made by cash, credit or debit cards.
4. All appointments and/or visits must be made and redeemed within the validity period.
5. All rates are based on per person basis and in Ringgit Malaysia.
6. The Package(s) are redeemable only by the named person and are non-transferable.
7. Additional treatments, medications, investigations, tests, or procedures required as a result of the Package(s) screening shall be invoiced separately and payable on the same day by cash, credit or debit cards.
8. Patient will only be refunded if found not fit for the programme, after deducting the charges incurred for all the services that have been performed. The refund amount shall be informed after completion of the 1st consultation by the Rehabilitation Medicine Physician.
9. The information provided is on an “As Is” and “As Available” basis without warranties of any kind and is subject to change without prior notice.
10. The decision and consideration made by SJMC is final and non-negotiable.

To be eligible to enroll in this programme, a Pre-Assessment will be conducted by our Rehabilitation Medicine Physician at RM235.
