  1. The free influenza vaccine is available at the Outpatient Centre, Subang Jaya Medical Centre (SJMC), from 23 August to 30 September 2021, for individuals aged below 18. 
  2. You are required to register your interest in a Google Form at https://forms.gle/2P3hE7Ta885pKaV17 
  3. A total of 64 individuals on a first-come-first-served registered basis will be eligible for the free influenza vaccine programme.
  4. SJMC shall contact successful applicants for booking of appointment and confirmation. 
  5. The offer is for the vaccine only and excludes the paediatrician’s professional fees.
  6. The free vaccine programme is not valid with any other privileges and/or promotions offered by SJMC.
  7. The free Influenza vaccine programme is not valid for RSDH employees.
  8. The management of SJMC reserves all rights to change or amend the Terms & Conditions without prior notice.

  1. The free Basic Dental Check with doctor’s consultation is available at KL International Dental Clinics (KLIDC), located at Level 1, Mediplex, Subang Jaya Medical Centre (SJMC), from 23 August to 30 September 2021, for individuals aged from 12 – 22 years old.

  3. A total of 64 individuals on a first-come-first-served registered basis will be eligible for the free Basic Dental Check with doctor’s consultation.
  4. KLIDC shall contact successful applicants for booking of appointment and confirmation.
  5. The free Basic Dental Check is not valid with any other privileges and/or promotions offered by SJMC and/or KLIDC.
  6. The free Basic Dental Check is not valid for RSDH employees.

  2. The free eye screening will comprise:
  • Examination By Optometrist
  • Visual Acuity
  • Auto Refraction
  • Slit Lamp Examination
  • Intraocular Pressure (IOP)
  • Fundus Photography

  2. The free Hearing Test provided is Pure Tone Audiometry (PTA).

  4. A total of 64 individuals on a first-come-first-served registered basis will be eligible for the free Hearing Test.
  5. SJMC shall contact successful applicants for booking of appointment and confirmation.

  7. The free vaccination programme is not valid with other on-going privileges and promotions offered by SJMC.
