To ensure the smooth processing of your request, kindly ensure that the following documents are prepared:
- Photocopy of patient NRIC front & back
- Consent for release of information / request letter
- Necessary supporting documents where applicable
- Processing fee payable at time of collection
- Photocopy of applicant’s NRIC front & back if applicant is not patient or insured
The estimated turnaround time for processing your request is approximately 2 to 3 weeks (working days), from the time we receive all the duly completed documents. Please note that the duration for generating the report may be extended based on the complexity of the specific request.
- Will I be notified when my report is ready?
Yes, we will notify you regarding the availability of the report through phone/SMS or email. Please ensure that you have provided your contact details to our staff. - Where can I collect my report?
You can personally collect the report at our Medical Report Service counter, located on the 2nd Floor of the Outpatient Centre during office hours. Alternatively, you can request to have the report posted to your current provided address through a courier service. - Can I ask someone to collect the report on my behalf?
Yes. Please provide them with an authorisation letter. This letter should clearly state your consent for the designated person to collect the report on your behalf. - Can SJMC post it to my insurance company /PERKESO (SOCSO) /EPF office?
No. To maintain confidentiality and adhere to the PDPA Act 2010, we will only release the report to the requestor who has provided valid consent or authorisation. This policy ensures the protection of your personal information and mitigates any potential disputes regarding proof of delivery. We appreciate your understanding and cooperation.
