Terms and Conditions
1. Super Seniors Club (SSC) membership programme is available at CAH Medical Centres Sdn Bhd (CMC) Registration No.: 201301008653 (1038495-A) network of hospitals:
a. Ara Damansara Medical Centre
b. Bukit Tinggi Medical Centre
c. ParkCity Medical Centre
d. Subang Jaya Medical Centre
2. SSC is open for Malaysians and Expats aged 60 and above with a permanent residence address in Malaysia.
3. Enrollment must take place at the hospital.
4. SSC membership card is a privilege card which gives card holders the benefits of enjoying discounts, privileges and events from time to time offered by CMC hospitals.
5. To enjoy the discounts and privileges, the SSC membership card must be presented during registration, during purchase and prior to making payment. No reimbursement or refund on previous or back-dated transactions if card holders fail to present the SSC card during registration.
6. Discounts are applicable for SELF-PAY customers upon full settlement of payment and are NOT applicable on doctor’s fees and other on-going promotions, discounts and / or privileges.
7. SSC membership card is non-transferable and is only for the exclusive use of the card holder only.
8. SSC is a lifetime membership programme. There is no membership joining fee, and membership renewal is not required.

10. SSC discounts and privileges are redeemable during the next visit from the date of successful registration.
11. Members can enjoy the same discounts and privileges across CMC hospitals.
12. If the card has been lost, stolen or damaged, kindly write in to inform CMC at sjmc.marketing@asia1health.com. for replacement and CMC shall reserve its rights to impose a nominal replacement fee of RM10.00 per card.
13. For the benefits and privileges offered by the participating merchants (if any), CMC merely provides a platform to members to enjoy the privileges and benefits of the products/services of the said merchants. The relationship between CMC and the participating merchants does not create or constitute partnership, a joint venture, or an agency relationship and CMC shall not be held responsible and liable in any manner whatsoever for any implications of the products/services provided by the participating merchants. 14. Please allow up to 48 hours for a response to any correspondence. If a response has not been provided to you within 48 hours, please try contacting us again.
15. Should any of the conditions and privileges are misused or abused, CMC is entitled to suspend and terminate the card holder’s right to use the SSC card without prior notification.
16. CMC reserves the right to terminate the programme, cancel and/or modify any of the SSC terms & conditions, benefits, and privileges without prior notice. Upon termination or cancellation, the SSC Card shall be nullified and invalidated and CMC shall have no obligation to extend any benefits and/or privileges under the programme.
17. CMC reserves the right to use and utilise all information obtained from or under or pertaining to this programme for marketing purposes and the card holders shall be deemed to have consented to CMC of the same.
18. The card holder shall notify CMC in writing at sjmc.marketing@asia1health.com. of any updates or change of address or any other contact information to enable CMC to notify the card holder of any information or updates on the programme.
19. The use of the card by the card holder automatically infers acceptance of the prevailing terms and conditions of the programme.
20. Any information which the card-holder provides will be kept confidential and and treated with care based on CMC’s privacy and personal data protection policy which is available on https://subangjayamedicalcentre.com/privacy-and-pdpa-policy.
21. With the usage of the card, the card holder is regarded to have given his or her consent and authorises CMC to collect, process, disclose and use his or her personal data within CMC Group for marketing purposes, except that disclosure will be made where required by applicable law or any governmental/regulatory authority or any valid legal process.
22. Each party shall comply with all applicable laws, statutes, regulations and each party’s Code of Business Conduct and policies relating to anti-bribery and anti-corruption throughout the term of this membership. In the event of any breach by a party, this shall be deemed as a material breach and the non-defaulting Party may, without prejudice to any other rights or remedies available, terminate this arrangement without any liability to the defaulting Party.
